BILLING AND INSURANCE

Payments

We accept checks, cash, Visa, Discover, MasterCard, and American Express.
Your co-pay is required at the time of the appointment.
If you do not have insurance, a minimum deposit is required.
Call our office to set up payment arrangements in advance.

Fees

Fees vary dependent upon the extent of the services and the tests performed. You will be responsible for fees which may be associated with medical records being released for transfer of care, applying for new insurance, disability benefits, or other medical forms. For more detailed information, call the office during regular business hours to speak with our staff.


Insurance

We will bill your primary and secondary insurance for you. You are ultimately responsible for the account even if you have insurance. You are also responsible for obtaining any insurance referral and prior authorization that may be required. Our staff will be happy to answer any insurance questions you may have.

We will need to get a copy of your insurance card (both front and back) in order to process your insurance. Please keep us updated on any insurance changes such as policy ID numbers, phone numbers, and addresses.
