We appreciate you as a patient and hope you find our staff friendly and knowledgeable. We have designed this web site to help our patients be prepared for their visits as well as educated so allergies and asthma will not get the best of them.
On the left are several headings which help you be prepared for your visits—whether this is your first visit or you are an established patient coming in for your annual evaluation.

a handout which indicates which articles may be helpful.
Your First Visit
We try to do allergy testing (see “Diagnosis and Treatment”) for patients on their
irst visit to save time and money and make every appointment effective. Please
read all of the information, paying particular attention to “Points of information
regarding your allergy appointment,” so when you come for your first appointment,
you can get allergy tested if the doctor deems it necessary.
Billing and Insurance
We have listed methods of payment, fees, and the insurances we accept. If your
insurance is not listed, please call our office to see what can be done for you.
Patient Prescription Assistance Programs

Rescheduling Appointments
If you cannot keep a scheduled appointment, please call our office as soon as possible to reschedule another one. Late cancellations or “no-shows” prevent others from being seen. To address this need, cancellations less than 48 hours in advance will result in a $25 fee. Missed appointments will result in a $60 charge. Note that these charges are the individual’s responsibility as insurances do not cover these charges.
Patient Forms
Please fill out the forms relevant to your visit and have them ready for your appointment. There are forms for new as well as established patients. We also have forms to release your records which can be filled out and faxed to us.
